Aperion Care Quality Assurance Nurse in Lincolnwood, Illinois

SUMMARY: The primary role of the Quality Assurance Nurse is to ensure the facilities who are below 3 stars in the CMS Five-Star Quality domain rating system or listed as Special Focus Facilities improve their domain ratings to a score of 3 stars or above. This position will primarily focus on the facilities who are below 3 stars in the CMS Five-Star Quality domain rating system or listed as Special Focus Facilities. The individual will complete systematic reviews and analysis of the MDS coding and clinical practices. This review will identify fundamental causes and lead to the development of an individualized action plan for each building. The action plan will have measurable goals that assist in building specific programs that facilitate improved quality within the facility. Ultimately, this position is responsible for ensuring the facilities Five-Star Quality domain rating achieves & maintains a score of 3 stars or higher.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

· Work alongside facilities’ clinical and operational teams to guide improvements that impact change throughout the CMS Five-Star rating system all while promoting team collaboration to maintain the highest quality of resident care.

· The facilities whose Five-Star Quality domain ratings are below 3 stars will have each their Five-Star domain assessed for risk and opportunity. This will be reviewed with the facility.

· Actively assist in identification of fundamental cause analysis for facilities whose Five-Star Quality domain ratings are below 3 stars. Support the MDS Coordinator and Interdisciplinary Team (IDT) with development of an action plan to addresses the root cause analysis findings.

· Work directly with the facilities in adjusting processes and building programs to maintain the highest quality outcomes with measurable results.

· Conduct monthly chart audits for all facilities whose Five-Star Quality domain rating is below 3 stars. The chart audits will include a review of items such as psychotropic drug review, admission documentation, foley catheter use, urinary tract infections, vaccine compliance, falls, wounds, ADL decline, locomotion decline, and discharges.

· Continuously evaluate CASPER reports for improvement opportunities associated with the quality Five-Star domain.

· Orientate MDS Coordinators and IDT on proper documentation practices, MD order follow-through, and meeting objectives.

· Identify MDS staff in-servicing needs & work with IDT to offer/arrange in-services as needed.

· Educate MDS Coordinator and IDT on the functionality of Ability Care Watch software and best practices for use.

· Provide ongoing staff education as needed to support established processes and programs and or reestablishment of processes and programs.

· Remain current and up to date on changes made to the Five-Star Rating System and effectively communicate and educate staff.
